I'm not able to diagnose but it sounds like it could be Rheumatoid arthritis. The fact it is affecting the smaller joints and symmetrically are the big clues.

The other possible explanation is some wear and tear in the vertebrae of the neck, impeding circulation or pinching on the nerves that run down the arms to the fingers.
If your doctor is able to confirm this then please consider trying some glucosamine for 8 weeks and applying some Atrogel® (arnica) to the fingers three to four times a day.
